Home-Based Therapist (Lake County)

Description

The Home Based Therapist position provides in person therapy services to individuals and families in the client's home within Lake County. This position will also exercise considerable independence of judgment in the application of therapy skills to an identified caseload of clients with individual and/or family problems requiring a variety of intervention and service techniques and skills.

Responsibilities

  • Provide individual and family mental health counseling to clients referred by the Department of Child Welfare
  • Evaluate, assess and diagnose clients based on presenting issues
  • Identify and set goals to meet client’s needs
  • Develop individualized treatment plan to support therapeutic interventions using identified goals
  • Consult with clinical team and participate in clinical team meetings
  • Maintain communication with referral source and Crown team pertaining to client case
  • Respond to client crisis and/or risk as needed
  • Attend team meetings, supervisory meetings and court
  • Demonstrates knowledge of mental health standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior

Qualifications

  • Master's of Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or any other equitable Master's degree in behavioral sciences
  • Preferred to have one of the following licenses in the state of Indiana: LSW, LCSW, LMFT, LMHCA, LMHC
  • Previous experience in mental health counseling
